These forms should be used for the majority of haematological malignancy tests, EXCEPT whole genome sequencing. Please note that these forms are not intended for use where samples for the diagnosis of a haematological malignancy are being referred to the local SIHMDS for morphological/immunophenotypic assessment. In these instances, the SIHMDS will subsequently refer on for appropriate genomic testing.
