If you require whole genome sequencing (WGS) please use our Haematological malignancy WGS form instead.
Please do not use these forms if you are referring your samples to the local SIHMDS for morphological/immunophenotypic assessment to diagnose haematological malignancy. In these instances, the SIHMDS will make the referral for appropriate genomic testing.
