Updated national genomics forms

Genomics forms managed nationally by NHS England, including order forms for whole genome sequencing (WGS), have been updated (6 May 2026). Please always check you are using the current versions.

The changes are intended to improve experience when completing forms electronically and align with the recent national genomic test directory (NGTD) updates. They have been informed by extensive national consultation with requesting clinicians.

You are encouraged to complete form electronically where possible. Full functionality requires use of Adobe Reader or Acrobat software.

Rare disease WGS order form (v2.0)

Notable changes to the form include:

  • Additional fields to validate the need for testing and expected clinical use of results.
  • Requirement to provide information on biological sex to aid quality control during testing.
  • Modification of the human phenotype ontology (HPO) table to include ‘Unknown’ as an option.
  • Updated list of panels in line with the latest NGTD updates.

Cancer WGS order form (v2.0)

Notable changes to the form include:

  • Simplification of solid tumour ordering, including removal of ‘Additional tumour information’, ‘Tumour topography’ and ‘Tumour morphology’ fields.
  • Additional checks when ordering “M232.1 Solid tumour exhausted all standards of care testing and treatment - Adult WGS” to ensure a working diagnosis/morphology (SNOMEDCT) is provided.
  • Updated list of panels in line with the latest NGTD updates.

WGS Record of Discussion forms (v4.04)

The form, including translations, has been improved to auto-populate information input on Page 1 to all other pages. The Romanian translation has been additionally updated to align formatting with all other translations.

Other forms

The Young persons’ assent (v3.03) and Consultee declaration (v3.04) – for those signing on behalf of others – are easier to navigate and complete electronically including additional tick boxes and electronic completion of the ‘Date of Signature’ field. In the Consultee declaration, the remote consent field has been moved into the patient consent section. More guidance on these forms is included on our consent pages.

The National genomic research library participant leaflet (v3.0) has also been revised by Genomics England.
